Thanks for requesting an appointment!

Thank you for booking your child’s dental appointment with Children’s Dental Health of Exton! A team member will be in touch with you shortly to confirm your appointment time.

What can I expect the day of my child’s dental appointment?

On the day of your scheduled appointment, a parent or legal guardian must be present with:

  • Photo ID or Driver’s License
  • Insurance Card
  • Appropriate custody or legal guardianship paperwork (if applicable)

If a parent or legal guardian is not available to attend your child’s appointment, you must submit your New Patient Paperwork in advance and list other person(s) authorized to bring your child. The parent or legal guardian must be readily available on the day of the appointment in case the office needs to reach out via phone.

What we’re doing to keep your family safe

Your family’s safety is our first priority. You may print and complete the consent portion in advance, but the questionnaire must be completed in the office by a member of our staff. Click below to download the COVID Pre-Screening Questionnaire. You’ll also notice additional safety protocols in place at your next appointment including required masks, temperature checks, our waiting buzzer system, additional sanitizing procedures, limited patients in the hygiene bay area, and more.
