What can I expect the day of my appointment?
On the day of your scheduled appointment, a parent or legal guardian must be present with:
- Photo ID or Driver’s License
- Insurance Card
If a parent or guardian is not available to attend your child’s appointment, you must submit your New Patient Welcome Packet in advance and list any other person(s) who are authorized to bring your child.
