If you’re filling out your Patient Paperwork within 24 hours of your appointment, please print a copy of your Patient Paperwork and bring it with you. There is a checkbox at the end of the form for a copy to be emailed to you.

If you are unable to complete your paperwork in advance, please arrive 20 minutes prior to your child’s appointment to allow time for completion.

Orthodontic Patient Paperwork

To fill out orthodontic paperwork, please download the form above and fill in the information on your computer and print it or print it and then fill it out by hand. Don’t forget to bring it with you to your appointment!

What can I expect the day of my child’s dental appointment?

On the day of your child’s scheduled appointment, a parent or legal guardian must be present with:

• Photo ID or Driver’s License
• Insurance Card
• Appropriate custody or legal guardianship paperwork (if applicable)

If a parent or legal guardian is not available to attend your child’s appointment, you must submit your New Patient Paperwork in advance and list other person(s) authorized to bring your child. The parent or legal guardian must be readily available on the day of the appointment in case the office needs to reach out via phone.

What we’re doing to keep your family safe

Your family’s safety is our first priority. You may print and complete the consent portion in advance, but the questionnaire must be completed in the office by a member of our staff.
